However, in the last few decades there has been a growing interest in the development of non-selective NSAIDs, including diclofenac and celecoxib.

NSAIDs exert their anti-inflammatory effects by inhibiting cyclooxygenase enzymes (COX and COX-1), including prostaglandins (PG) (prostacyclin) and (in the case of prostaglandin E2) (see section 2). Although diclofenac is one of the most widely used NSAIDs, diclofenac sodium and celecoxib are widely used by the elderly. However, the safety and efficacy of these drugs have not been adequately studied in the elderly population.

To understand the safety and efficacy of non-selective NSAIDs, it is necessary to analyze the safety profile of the most commonly used NSAIDs. This review article, focusing on diclofenac, shows the relationship between cardiovascular risks and drug interactions.

Diclofenac is a selective cyclooxygenase (COX) inhibitor, with the pharmacologic activity of blocking prostaglandin formation from the COX isoform of cyclooxygenase (COX-1). COX-1 is found in the liver, intestines and other sites of synthesis in the gastrointestinal tract. As a result of inhibition of COX-1, prostaglandins increase through inhibition of the COX-2 isoform of the enzyme, resulting in the production of prostacyclin.
